HIPAA Notice of Privacy Practices

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

This Notice applies to My Harmony Now, LLC and Cammie Broussard, PMHNP (collectively, the "Practice," "we," "us," or "our"). We are required by law to maintain the privacy of your protected health information ("PHI") and to provide you with this Notice of our legal duties and privacy practices.

Our Role and the Role of Our Technology Partner

My Harmony Now, LLC is the covered entity responsible for your care and your PHI. We use Headway (Therapymatch, Inc. d/b/a Headway) to provide non-clinical administrative support — scheduling, our secure patient portal, secure telehealth video visits, insurance verification, and billing/claims submission — under a Business Associate Agreement. Headway does not provide professional healthcare services; all clinical care is provided directly by Cammie Broussard, PMHNP.

How We May Use and Disclose Your Health Information

We may use and disclose your PHI, without your written authorization, for:

Treatment: To provide, coordinate, or manage your psychiatric care, including sharing information with pharmacies or other providers involved in your care.
Payment: To bill and collect payment, including submitting claims to your insurer and processing payments through Headway.
Health Care Operations: For quality assessment, compliance, and business planning necessary to run our Practice.

Other Permitted or Required Uses (Without Authorization)

Public health activities as required by Louisiana law
Health oversight activities (audits, investigations)
Judicial and administrative proceedings (court order or subpoena)
Law enforcement purposes, as required by law
To avert a serious threat to health or safety
Workers' compensation, as authorized by law
Coroners, medical examiners, or funeral directors as necessary
Research purposes, under strict privacy safeguards

Uses Requiring Your Written Authorization

We will not use or disclose your PHI for marketing, sale of PHI, disclosure of psychotherapy notes, or any other purpose not described above without your written authorization. You may revoke a prior authorization in writing at any time, except to the extent we have already relied on it.

Electronic Communications and Telehealth

Your care is delivered via secure telehealth video visits through our patient portal on the Headway platform. If you communicate with us by ordinary email or text message outside the secure portal, these methods carry inherent privacy and security risks — messages could be intercepted or misdirected, and are not treated as secure communications. Emails or texts you send may become part of your medical record. Do not use email or text for urgent or emergency situations. If you are experiencing an emergency, call 911 or the 988 Suicide & Crisis Lifeline.

Your Rights Regarding Your Health Information

Request Restrictions on how we use or disclose your PHI.
Request Confidential Communications at a certain phone number or email.
Inspect and Copy your medical and billing records, generally within 30 days of your request.
Request Amendment if you believe your health information is inaccurate or incomplete.
Receive an Accounting of Disclosures made in the six years prior to your request (with certain exceptions).
Obtain a Paper Copy of this Notice at any time.
File a Complaint with us or with the U.S. Department of Health and Human Services, Office for Civil Rights, at www.hhs.gov/ocr/privacy/hipaa/complaints or by calling 1-800-368-1019. We will not retaliate against you for filing a complaint.

Breach Notification

In the event of a breach of your unsecured PHI, we will notify you as required by the HITECH Act and applicable Louisiana law.

Changes to This Notice

We reserve the right to change this Notice and to make the revised Notice effective for PHI we already maintain, as well as information we receive in the future. The current Notice is always available on our website and at your appointment.
